The Case of Jeff: Pedophile in Institution
The case involves Jeff, a 35-year-old male inmate in a maximum security after he was convicted for being a pedophile. Prison gang members who are aware of the crime he committed make him a target for homosexual activity which he cannot resist since such information can make his life in a prison more complicated. In addition, giving in to their requests secures him protection in the facility. Jeff has taken this option since prison can be a treacherous environment especially when being targeted by gangs and thus he has the desire to survive. However, this has raised health and sexual identity concerns.
The confusion about his sexuality can lead Jeff into engaging in unhealthy sexual activities after he is released from prison. This is mainly attributed to the fact that he was not resistant to the sexual assault. Thus it is likely that after sometime he got used to the habit. Jeff might also revert to being a pedophile after he leaves the maximum security facility. The challenges that Jeff is experiencing can be linked to the rape experiences that he has put children through. A perpetrator is never sure whether he has enjoyed the experience.
As his case manager, it is important to ensure that all his problems are resolved so that he can see things the right way. The main objective of case management is to ensure that all the underlying issues that led Jeff to be a pedophile and those that made him agree to sexual assault are addressed. Being a pedophile, it is likely that Jeff does not understand and has not experienced a healthy normal relationship. In addition to unhealthy sexual experiences, Jeff could be having psychological problems. He may feel guilty for being a pedophile and for the sexual activities he is having in prison could cause distress. Therefore, his case manager should assess him and use a treatment program that restores his mental health and his ability to be in healthy adult relationships (Ccmcertification.org, 2017).
One of the best intervention strategies is the use of evidence based practice. There is an increase emphasis on the use of this practice in case management. This practice can be used to treat mental health victims, drug and substance abuse users, and sex offenders among many others. Case managers working with offenders in the criminal justice system are using this strategy more due to its effectiveness. The practice guides case managers and informs them on the progress achieved by delivering the treatment and services. The program integrates components such as client value and services, current client needs, environment and situation, best available research evidence and clinician’s expertise and judgment.
Evidence based practice requires that a case manager, researcher, client and prison officers to work together to identify and determine the best treatment programs that suit a client. The practice ensures that the development and implementation of case management services produces effective outcomes in order to use this practice, time should be taken to collect data in a certain area which is them incorporated with client information in order to get the best results.
In addition, client empowerment is a very powerful intervention practice in case management. A case manager’s job is to help their client to become the best version of themselves. In the criminal justice system, case managers help offenders with treatment programs that suit their needs so that they can become productive members of the society upon their release. They address issues such as anger, drug and substance abuse, mental health and sexual health among others. Some of these issues are the major reasons as to why most clients commit crime. Treatment programs restore their health and ensure that they do not reoffend. The offenders are empowered to reach higher levels and make healthy choices (Colonnieves, 2016).
Cognitive behavioral therapy can be used to treat a wide range of offenders. The theory is based on the fact that a person’s thoughts affect the way they feel and act (McLeod, 2008). Negative thoughts often lead to bad judgment and bad choices. In addition to residual programs and therapeutic communities, cognitive behavioral therapy can be effective in treating sex offenders. The focal point of the program is to change a sexual offender’s pattern of thinking in relation to their sexual behavior and interest. A wide range of treatment approaches such as behavior skills training, empathy, assertiveness, and conditioning based approaches are used. This can be a suitable treatment approach for Jeff. This is because it will restore his social skills, emphasize on victim empathy, sex education, relapse prevention, arousal reconditioning, and anger management.
In conclusion, Jeff’s mental health has been undermined by his emotional, health, sexual and psychological experiences. Intervention measures should continue even after he is released to ensure that he retains his sobriety. He should go for HIV test as soon as he is released so that he can know the way forward. His sexual identity issues can be resolved through counseling services that are tailored to meet his needs. By the end of the treatment programs, his chance of reverting to his former pedophile behavior should reduce and he should be able to have normal adult-adult relationships.
